Child Medical Statement

 This form is required to be completed and returned by August 15 for fall enrollment and December 15 for January enrollment for New Families.  It must be completed by your child's doctor's office.  The exam date must be less than 1 yr prior to the start of school. The vaccination record must also be attached.  This form has to be updated each year based on exam date. You can email the completed form and immunization record to [email protected].
Tip: Make sure that it is signed and date of examination written. Also make sure the practitioner completes Section B AND attaches the immunization record.

Child Medical/Physical Care Plan

This form is needed IF your child has

•  a medical condition that requires staff to monitor, 
• your child must avoid certain foods or activities,
• or requires emergency medications. 

This form is a fillable pdf. Once you complete it, you can download and email it to [email protected].

Authorized Pick Up

This form tells us who is allowed to pick up your child. If a parent is not allowed to pick up, you will also need to provide a copy of the court order. This is an online form.

Image Consent

 We take photos to document your child's preschool experience.  If we want to use a photo for a reason not on the form, we will always contact you to ask for permission. This is an online form.

Automatic Tuition Payments

Never have to think about tuition payments by having them automatically withdrawn from your account on the 15th of the month. This form is optional. If you prefer a pdf to print, complete, and mail in, let the director know by emailing [email protected]. Form must be completed by August 21st to ensure the first payment (Sept 15) will be able to be withdrawn.
